General Request Guidelines
Your room reservation request will remain PENDING until we review and approve. After your request is approved, you will receive a CONFIRMED/APPROVED email which will include additional information regarding payment, room/space
setup, staff contact details, etc.

- Within 5 business days of request - A $50 non-refundable deposit is due and the request will be reviewed by our staff for approval
- Within 5 business days of approval - A $200 refundable damage deposit is due
- Within 14 calendar days of approval - A Certificate of Insurance is due
- Within 20 calendar days of approval - Signed agreement and 1/2 of the rental fee is due
- By no later than 10 days before event start day - remaining rental fee is due
General Facility Use Guidelines
1. Rental Times: Can begin as early as 7:00 a.m. and must end by 10:00 p.m. with personal and rental equipment
removed from facility by 11:00 p.m.
2. Gym Package Includes: Use of Gym and Foyer. Tables and chairs are not included in package.
3. Gym/Stage Package: Use of Gym and Stage, includes tables and chairs
4. Price Includes: Set up and takedown of tables and chairs, normal event clean up and on-site facility staff.
5. Take Down/Cleaning: Package prices include table and chair take down and facility cleaning. Sufficient time should be
allocated for removal of personal and rental equipment and should be done within time allotted in package.
6. Caterer Services: Option to choose from Dr. Ephraim Williams Family Life Center Preferred Caterer’s List.
7. Building Security: Is required for most events during the time when guests are present. This cost is in addition to the
rental rate. Most events require Dr. Ephraim Williams Family Life Center Security Services or private security. Dr. Ephraim Williams Family Life Center staff will make necessary security arrangements. If security is required to provide additional service beyond scheduled time, user will be responsible to pay for additional time.
8. Insurance Requirements: A Certificate of Liability Insurance is required. See “Insurance Requirements” for further
information.
9. Candles, Pyrotechnics or open flames: are not permitted at or on the Dr. Ephraim Williams Family Life Center.
10. Smoking: Absolutely NO smoking of any form in or around Dr. Ephraim Williams Family Life Center.
11. Alcohol: Absolutely NO alcohol of any form to be used in or around Dr. Ephraim Williams Family Life Center.
12. Responsibility: Dr. Ephraim Williams Family Life Center staff is not responsible for any items left at the facility after the
scheduled event has concluded.
13. Decorations: All decorations must be done by user. Set-up/Decorating Time: Must be done within time allotted in package. Additional set up/decorating time can be purchased at $100.00 per hour.
Storage facilities are not available for User (this includes caterer’s equipment or special rental items such as tables to be stored before or after the event.). No staples, tacks, nails or screws may be used on fixtures, windows or ceilings. Glitter, hay, straw, silly string, or rice is prohibited inside all facilities. Birdseed is for outside use ONLY.
Balloons must be secured and weighted when utilized in the facilities and must be removed by the group immediately following the event. No free-floating balloons are permitted inside the buildings. If an outside rental company provides tables, chairs, or other equipment for the event, the user is responsible for setup and takedown of this equipment.

Payment and Deposit Guidelines
Reservation deposit: A reservation deposit of $50.00 is required to reserve an event date. The user has (5) business days to confirm reservation. When reservation is confirmed the $50.00 will be counted towards the total cost of the rental. If reservation is not confirmed the deposit is forfeited.
Facility/equipment damage and/or time overage: A $200 damage/overage deposit is required. The deposit is due (5) business days after confirmation of reservation. If paid with a seperate check, this $200 damage deposit check will not be deposited. And, if no damages, then the original check will be returned to the user within (10) business days after event.
50% Payment: Within twenty (20) business days from rental request date the user will be responsible for ½ of the total event cost.
Final Payment: The balance of the rental fee is due ten (10) business days prior to the event date.

- We accept all major credit/debit cards, as well as, cashier checks, money orders, and personal/business checks.

- You may mail or hand-deliver payments during our normal center hours

Cancellation and Change Guidelines
Cancellation fees are as follows:
2 to 6 months before event 40% cancellation fee
1 to 2 months before event 60% cancellation fee
Less than 1 month before the event Full payment is forfeited
A date change can occur with a $100.00 processing fee if notice is given at least 20 business days prior to the
event date.
